These Terms and Conditions apply to appointments, consultations, treatments and related services provided by Revive Weight Loss Clinic (“we”, “us” or “the Clinic”).
Our registered or principal business address is 18 Elmfield Road, Gosforth, Newcastle upon Tyne, NE3 4BP. You can contact us by telephone on 01912841355 or by email at info@reviveweightlossclinic.co.uk.
Please read these Terms and Conditions before booking an appointment.
1. About these terms
By booking or attending an appointment, you confirm that you have read and accepted these Terms and Conditions.
Your booking and treatment contract is with Revive Weight Loss Clinic. Our website uses Bookly to facilitate online bookings, but Bookly does not provide your consultation or treatment.
Nothing in these terms affects your statutory rights as a consumer.
2. Booking an appointment
Appointments may be requested through our website, by telephone or by contacting the Clinic directly.
An online booking is not confirmed until you receive a booking confirmation from us. Please check the details in your confirmation and contact us promptly if anything is incorrect.
You are responsible for providing accurate and complete contact information. We may use the telephone number or email address you provide to send appointment confirmations, reminders or important information concerning your appointment.
Appointment reminders are provided as a courtesy. You remain responsible for attending or cancelling your appointment on time even if a reminder is not received.
3. Prices and payment
No payment is taken through our website when you make an online booking.
The price of your consultation, treatment or other service will be displayed or explained before the service is provided. Unless we agree otherwise, payment is due at the Clinic following the appointment/before treatment begins.
If additional consultations, tests, products, prescriptions or treatments are recommended, their cost will be explained to you before you agree to proceed. You are under no obligation to purchase an additional service or product.
4. Cancelling or rescheduling an appointment
If you need to cancel or reschedule an appointment, you must notify us at least 48 hours before the appointment’s scheduled start time.
You can cancel or reschedule by:
- using the cancellation or rescheduling link in your booking confirmation, where available;
- calling us on 01912841355; or
- emailing info@reviveweightlossclinic.co.uk.
Electronic cancellation notices are treated as received at the time they reach our booking system or email inbox.
A cancellation made less than 48 hours before an appointment is a late cancellation. Failure to attend without giving notice is treated as a no-show.
For a late cancellation or no-show, we may:
- charge a cancellation fee; and/or
- require any outstanding cancellation fee and payment in advance before accepting another booking.
Any cancellation fee will be reasonable and will reflect the time reserved, preparations made and loss incurred by the Clinic. We may waive it where there are exceptional circumstances, at our reasonable discretion.
This policy does not restrict any cancellation or other rights you may have under applicable consumer law.
5. Late arrival
Please arrive on time for your appointment.
If you arrive late, we will try to provide the appointment within the remaining time. However, it may be necessary to shorten or reschedule it where there is insufficient time to provide the service safely or without affecting other clients.
If an appointment cannot proceed because of a late arrival, it may be treated as a late cancellation and the charge described above may apply.
6. Cancellation by the Clinic
Occasionally, we may need to cancel, postpone or change an appointment because of staff illness, safety concerns, equipment problems or circumstances outside our reasonable control.
We will give you as much notice as reasonably possible and offer an alternative appointment. We will not charge a cancellation fee where we cancel the appointment.
We are not responsible for reasonable travel or other indirect expenses incurred as a result of a changed appointment, except where the law requires otherwise.
7. Medical suitability and clinical decisions
Our services are subject to an individual assessment. Booking an appointment does not guarantee that a particular treatment, medication or programme will be suitable for you.
All prescribing and treatment decisions are made by an appropriately qualified healthcare professional using their independent clinical judgement. We may decline, postpone, change or discontinue a treatment where we reasonably believe that doing so is necessary for your health or safety.
Where treatment cannot proceed, we will explain the reason where it is appropriate and lawful to do so.
The Clinic is not an emergency medical service. If you have a medical emergency, call 999 or attend an appropriate emergency department. For urgent medical advice that is not an emergency, contact NHS 111 or your GP.
8. Information you provide
You must provide complete and accurate information about your health, medical history, symptoms, medication, allergies, pregnancy or breastfeeding status and any other matter that could affect your treatment.
You must tell us if this information changes during your care.
Withholding relevant information or providing inaccurate information may make treatment unsafe. In those circumstances, we may decline or discontinue treatment.
You must follow the instructions and safety advice provided by your clinician. If you experience unexpected symptoms or side effects, seek appropriate medical advice and contact the Clinic using the instructions given to you.
9. Treatment expectations and results
We will provide our services with reasonable care and skill. However, individual responses to weight-management programmes, treatments and medication differ.
We cannot guarantee a particular amount or rate of weight loss, or any other specific result. Information, testimonials, photographs and general examples on our website illustrate possible experiences but do not promise that you will achieve the same outcome.
Treatment results may be affected by matters including your health, adherence to clinical advice, diet, physical activity, other medication and individual biological factors.
10. Prescriptions, medication and products
Medication will only be prescribed when a clinician considers it clinically appropriate and legally permitted.
A recommendation made during an initial enquiry or booking is not a prescription and does not guarantee that medication will be supplied.
Availability may be affected by manufacturer or supplier shortages. Where a prescribed product is unavailable, we will discuss appropriate options with you. We will never substitute prescribed medication without suitable clinical approval.
For health and safety reasons, medication or sealed healthcare products that have been supplied to you may not be returnable or reusable unless they are faulty, damaged, incorrectly supplied or the law requires otherwise.
11. Your conduct
We expect clients and staff to treat one another with courtesy and respect.
We may end an appointment or refuse future bookings where a person behaves in a threatening, abusive, discriminatory or otherwise inappropriate manner. Any amount properly due for services already provided will remain payable.
12. Website information
Content on our website is provided for general information and is not a substitute for personalised medical advice, diagnosis or treatment.
Although we take reasonable care to keep information accurate and current, medical guidance, service availability and prices may change. If website information conflicts with advice given to you by your clinician about your individual care, you should seek clarification from the Clinic.
13. Bookly and other third-party services
Our website uses Bookly or related technology to process appointment requests and send booking communications.
We are not responsible for temporary interruptions or technical faults affecting a third-party booking platform. If you are unsure whether a booking, cancellation or rescheduling request has been received, please contact us directly.
14. Privacy and confidentiality
We process personal information, including health information, to manage bookings and provide safe and appropriate care.
Details of how we collect, use, store and share personal information, and information about your data-protection rights, are set out in our Privacy Policy, available here.
Clinical confidentiality may be subject to limited legal and safeguarding exceptions, as explained in our Privacy Policy.
15. Complaints
If you are dissatisfied with our service, please contact us so that we can try to resolve the matter.
Complaints should be sent to info@reviveweightlossclinic.co.uk.
We aim to acknowledge complaints within 5 working days and provide a substantive response within 10 working days, where reasonably possible.
Details of any applicable independent complaints or regulatory process can be provided on request.
16. Our responsibility to you
We are responsible for losses that are a foreseeable result of our breach of these terms or our failure to use reasonable care and skill.
We are not responsible for loss that could not reasonably have been foreseen when the contract was formed, or for business losses arising from services purchased wholly for personal use.
Nothing in these terms excludes or limits liability where it would be unlawful to do so. This includes liability for death or personal injury caused by negligence, fraud or fraudulent misrepresentation, and your statutory consumer rights.
17. Changes to these terms
We may update these Terms and Conditions from time to time – for example, where our services, booking arrangements or legal obligations change.
The terms applying to your appointment will normally be those in effect when the appointment is booked. Any change that materially affects an existing booking will be communicated to you where reasonably practicable.
18. Governing law
These Terms and Conditions are governed by the laws of England and Wales.
You may bring legal proceedings in the courts of England and Wales. If you live in Scotland or Northern Ireland, you may also be entitled to bring proceedings in your local courts.
